CMHRP provides care management for Medicaid eligible women during pregnancy and for the 60 days postpartum. Non-Medicaid patients may also be eligible for CMHRP services depending on their needs. CMHRP provides community resources to meet the mother’s needs, and help achieve the best possible birth outcome.
